This page explains, in plain language, what information the Salem Memorial Hospital Foundation collects on smdh.foundation and how we use it. The Foundation’s legal name is Salem Memorial District Hospital Foundation, a 501(c)(3) nonprofit (EIN 83-1617383).
What we collect
- Orders (golf teams, sponsorships, dance tables and seats, gifts): your name, email, phone, billing address, what you bought, team or guest names, and any notes you add.
- Donations (Become a Donor form): your name or business name, email, gift amount, whether you want to be listed or stay anonymous, any “in honor of / in memory of” name, and, if you ask us to send a note, the recipient’s name and email.
- Dancer sign-ups: your name, email, phone, partner’s name and any notes.
- Sponsor logos: logo files you upload for signs, the program or our Thank You page. Logos and names of sponsors and supporters who choose to be listed are shown publicly.
- Email: messages you send us, and the order confirmations, receipts and replies we send you.
Payments
Card and Cash App Pay payments are processed by Square. Your card details go directly to Square; we do not see or store your full card number. Square’s privacy policy is at squareup.com/legal.
How we use it
To run our events, fill your order, send receipts and tax receipts, recognize sponsors and donors who want to be listed, pair dancers, and answer your questions. Staff and volunteers who run the events can see the details they need.
Cookies and analytics
The site uses cookies to keep your cart and checkout working. We also use advertising and analytics pixels from Reddit and Snapchat, which can set cookies and tell those companies that a browser visited our pages, so we can measure and reach people interested in our events. You can block these with your browser’s cookie or tracking settings, or an ad blocker; the site still works without them.
What we don’t do
We do not sell or rent your information. We share it only with the services needed to run the site and take payments (such as Square and our web host), or when the law requires it.
Your choices
You can ask us to see, correct or delete your information, or to remove your name or logo from public recognition. We keep order and gift records as long as tax and accounting rules require.
