These terms cover purchases and gifts made on smdh.foundation, run by the Salem Memorial Hospital Foundation (legal name: Salem Memorial District Hospital Foundation, a 501(c)(3) nonprofit, EIN 83-1617383).
Payments
We accept card and Cash App Pay through Square. We do not take checks online. A Service charge is added to event tickets and sponsorships so the Foundation receives the full amount after processing fees. On donations, covering processing costs is optional.
Donations and taxes
Gifts are tax-deductible as allowed by law. For tickets, tables and sponsorships, only the amount above the fair market value of what you receive may be deductible. A receipt is emailed after your gift. Please ask your tax advisor about your situation.
Refunds
Event tickets and sponsorships are non-refundable but transferable; if an event is canceled, purchases are refunded or may be converted to a donation. The Service charge covers processing and is not refunded unless the event is canceled.
To transfer a ticket or team spot, or to ask about a refund, email support@smdh.foundation.
Events
Dates, times, prizes and seating can change; we post updates on the event pages and email ticket holders. Sponsor logos and names must be yours to use, and we may decline any that aren’t suitable for a family event.
